BRIDGE

BRIDGE is a community-based outreach program serving adults 18 and over in Gloucester County with a serious mental illness and legal justice involvement. The program supports community integration by connecting individuals to resources that promote stability, independence, and ongoing support.

Eligibility

To be eligible for BRIDGE services: 

  • Be 18 years of age or over
  • Be involved in the legal justice system 
  • Have a serious and persistent mental illness, primarily schizophrenia, psychotic disorders, major depression, bipolar disorder, delusional disorder, and schizoaffective disorder 

What to Expect 

  • Reaching Out

    Individuals can be referred from other Acenda programs, state or county hospitals, the legal justice system, or other community providers by emailing glcasemanagement_referrals@acendahealth.org. Clients can also call the Acenda Access Center at (844) 422-3632 ext. 9500. 

  • Arranging Services

    Individuals will be contacted by phone within 48 hours of referral and will begin services immediately, including completion of intake assessments.

  • Moving Forward with Care

    Individuals will meet with their case manager in person on a weekly basis to begin linking to services and setting their self-defined goals. 

FAQ’s

How can you help me with my immediate issues? 

BRIDGE provides community resources based on an individual’s needs and goals. Case managers work with the individual and Salem County Jail, probation, and all other relevant legal services.

How can I get signed up for services? 

You can be referred from other Acenda programs, state or county hospitals, emergency screening services, shelters, community providers and DMHAS-contracted diversion beds. You can also self-refer.

How much do BRIDGE services cost? 

BRIDGE services are free.

How long will I be working with the program? 

Typically, individuals remain in the BRIDGE program for 3-6 months, but in some cases, you can remain with BRIDGE for 12 months.

Where do sessions take place? 

Sessions are typically in-person and take place at the following locations: at home, in the community, jail, probation office, or at an Acenda office.

Can you help me navigate the mental health system?

Yes, BRIDGE case managers can assist individuals with navigating Acenda’s internal resources and external community resources.  Case managers can provide linkages to resources to address individual concerns.

Who is eligible for services?

  • Must be an adult 18+ 
  • Be involved in the legal justice system 
  • Have a serious and persistent mental illness, primarily schizophrenia, psychotic disorders, major depression, bipolar disorder, delusional disorders, and schizoaffective disorder

Are there any exclusions to receive care?

Individuals with the following conditions or circumstances are not eligible for program services:

  • State-placed inmates
  • A primary diagnosis of a developmental disability or substance use
  • Cognitive conditions such as delirium, dementia, amnesia, brain injury, or other cognitive impairments
  • Challenges primarily related to life circumstances rather than a mental health diagnosis, including relationship concerns, experiences of abuse or neglect, grief or loss, school- or work-related stress, or identity-related concerns
  • Individuals currently serving a sentence for, or charged with, a felony
