Nurse-Family Partnership

Acenda’s Nurse-Family Partnership (NFP) program is a free in-home prenatal nursing service designed to support first-time parents as they endure pregnancy and early childhood with their child. Available to residents of Atlantic, Cape May, Cumberland, Gloucester, and Salem counties, this program will provide a personalized nurse help you feel confident and supported throughout your pregnancy and parenthood experience.

    Identifying and treating anxiety.

  • Homelessness 

    Assisting you in your search for a temporary or permanent residence.

    Helping you create a healthy, nurturing environment for your family and supporting you through your parenting journey.

    Connecting you to important resources to guide you through your wellness journey. 

Eligibility

To be eligible for Nurse-Family Partnership, you must: 

  • Be in your first pregnancy 
  • Enroll in services prior to 28 weeks gestation  

What to Expect 

  • Reaching Out

    Contact the Connecting New Jersey program within your county to request services, after which they will connect you with a Nurse Home Visitor to work with you. Services can begin as soon as you reach out.   

  • Building a Partnership

    You will meet with your Nurse Home Visitor to review the program, discuss your needs, and determine your care plan.  

  • Moving Forward with Care

    You and your Nurse Home Visitor will meet weekly, moving to bi-weekly and then monthly meetings as you transition from pregnancy to parenthood. Your Nurse Home Visitor will be there to support you from the early stages of your pregnancy (prior to 28 weeks) through your child’s 2nd birthday. .

How much does the Nurse Home Visitor cost? 

The Nurse Family Partnership program comes at no cost to you.

Can my nurse come to the doctor with me?  

Yes, as long as they are available, your Nurse Home Visitor is welcome to attend doctor’s appointments with you.  

How does a home visiting nurse help during my pregnancy? 

Your Nurse Home Visitor will provide you with resources to support you in other areas of parenthood and life, while guiding you through your first pregnancy and parenthood experience. 

Is my maternal health visit private? Or does my partner, parent, or family need to be present? 

Your Nurse Home Visitor is there to serve the mother. You can decide who gets to participate in visits, and to what extent. 

Do I need a referral to get a Nurse-Family Partnership home visiting nurse?

Will my nurse conduct any in home assessments? 

Yes, your nurse will assess expecting moms during each visit using the NFP Strengths and Risks Framework.

Are services provided in-person or virtually? 

Preferably visits take place in person through home visits or at the location of the client’s choosing. Telehealth is an option, although the preferred method is face to face.
