Integrated Case Management Services

Integrated Case Management Services (ICMS) serves adults 18 and older who reside in Gloucester or Cape May counties. ICMS engages, supports, and empowers individuals living with serious mental health conditions to integrate into their communities, access available resources, and maximize their independence.

Eligibility

To be eligible for ICMS, you must: 

  • Be over the age of 18 
  • Have a serious and persistent mental illness, including: Schizophrenia, Psychotic disorders, Major depression, Bipolar disorders, Delusional disorders, Schizoaffective disorders, Severe personality disorders, if at high risk of hospitalization

What to Expect 

  • Reaching Out

    Individuals can be referred to ICMS through any other Acenda program, state or county hospital, screening services, shelters, community providers, and DMHAS-contracted diversion beds. Clients can also self-refer by contacting Acenda’s Access Center at 833-5842-9165. You can also email requests to glcasemanagement_referrals@acendahealth.org for Gloucester County, or cmcasemanagement_referrals@acendahealth.org for Cape May County.

  • Initial Meeting

    In your initial meeting, you will participate in treatment planning and goal development with an ICMS staff member. You will also take the GAD, PHQ, and SAFE-T assessments, and be linked to mental health and medication management.

  • Moving Forward with Care

    Services are provided in person either at the client’s home, in the community, or at an Acenda location. Services typically last between 3-6 months, or up to 12 months, depending on the client’s needs.  

FAQ’s

How can you help me with my immediate housing issues?

ICMS provides the most current and available community resources that an individual would qualify for in order to address housing needs. Case Management can refer individuals to the County Board of Social Services, HUD, the Housing Authority, etc. For linkages to address housing needs.

How long will I be working with your program? 

Typically, individuals remain in the ICMS program for 3-6 months, but in some cases, individuals can remain with ICMS for up to 12 months.

Can you help me navigate the mental health system?

Yes, ICMS Case Managers assist individuals to navigate Acenda’s internal resources and external community resources. Case Managers can provide linkages to resources that consumers need to address individual concerns.

How do I qualify for ICMS?

To qualify for ICMS, you must: 

  • Be over the age of 18

Can family members or other supports be involved in my care?

Yes. With your permission, family members, caregivers, or other trusted supports can be included in treatment planning and service coordination. Working together can strengthen your support system and help everyone better understand your goals while respecting your privacy and confidentiality.

What kinds of goals can ICMS help me achieve?

Your case manager will work with you to create goals based on your individual needs. Goals may include improving mental health, accessing healthcare, obtaining housing, applying for benefits, finding employment, managing medications, strengthening daily living skills, or connecting with other community resources that support long-term stability.

How often will I meet with my case manager?

The frequency of meetings depends on your individual needs and treatment plan. Some individuals require more frequent support at the beginning of services, while others meet less often as they make progress toward their goals. Your case manager will regularly review your needs and adjust services accordingly.

Do I need to be receiving therapy or medication management to participate in ICMS?

Not necessarily. If you are not currently connected to behavioral health services, your ICMS case manager can help coordinate referrals for therapy, psychiatric care, medication management, or other supports that align with your treatment goals. The program focuses on connecting you with the services you need to achieve stability.

Can I continue receiving services from other providers while enrolled in ICMS?

Yes. ICMS is designed to coordinate care, not replace existing services. Your case manager works alongside your healthcare providers, therapists, psychiatrists, and other community agencies, with your permission, to help ensure your care is coordinated and your goals are supported across all aspects of your treatment.
