LifeSet
The LifeSet Program supports young adults transitioning out of foster care or other child welfare systems by pairing them up with a caring LifeSet specialist who supports them with achieving personal goals. These goals may include finding housing, completing school, developing job skills, and creating healthy relationships, all with the focus of building stability, independence, and confidence as they move into adulthood.

Eligibility
To be eligible for LifeSet, you must:
- Be a young adult between 17-23 years old
- Be a young adult currently or previously in foster care or other child welfare system placements
- Must live in or be connected to one of the listed counties served
- A referral or pre-screening is required, followed by a pre-enrollment assessment to determine if the LifeSet is the right fit
What to Expect

Connecting with Our Team
Once LifeSet receives a referral from the Department of Children and Families (DCF), the young adult and their guardian will be contacted within 24 hours, and they will be matched with a LifeSet Specialist.

Initial Meeting
During the initial meeting, the young adult and LifeSetthe specialist will discuss their areas of focus (housing, school, employment, etc.) and assess the needs and priorities. The specialist will meet with the client in person at least once a week and is available by phone call, text message, and email in between meetings.

Moving Forward with Care
The LifeSet specialist will work alongside the young adult to support action steps like applying for employment, completing applications, securing housing and connecting them to community resources. The program typically lasts 6 to 9 months but is tailored to each individual’s needs.

How often will I meet with my LifeSet specialist?
You will meet with your LifeSet Specialist in-person at least once a week. Your specialist is also available by phone, text message, or email between scheduled visits.

What does LifeSet help with?

No, there is not a cost for LifeSet.
Is LifeSet mandatory?
LifeSet is voluntary. Young adults decide what areas of their life they want to work on, and their specialist helps them get there.
