Connect to Home

Connect to Home is a supportive housing program designed for young adults, aged 18 to 21 at time of intake, who are currently or have previously been in the custody of the Child Welfare System in Burlington County. The program provides participants with a housing voucher to be used within Burlington County, helping them secure stable and affordable housing. Each participant is also partnered with a Care Manager and Peer Advocate, who offers guidance and support in finding housing and developing the life skills needed to successfully maintain independent living.

Eligibility

To be eligible for Connect to Home services, you must: 

  • Be between 18 to 21 
  • Be currently involved or have been in the custody of DCP&P Burlington Local offices 

What to Expect 

  • Getting Started

    Once your referral is received by Connect to Home and your DCP&P case worker has completed the case conference, you will be contacted by phone and/or email to schedule a meet and greet.

  • Initial Meeting

    Your care manager will complete your consent forms, the Homeless Management Information System (HMIS) intake assessment, and the initial assessment during your first meeting. After doing so, you will begin to identify towns and apartment complexes that would be suitable for you within Burlington County.

  • Moving Forward with Care

    You will continue to meet with your care manager and peer advocate once a week for at least an hour for as long as you need.

What should I do to support my housing application? 

Maintaining a source of income is a great way to support your housing application.

How often do I have to meet with my Connect to Home case manager?

You must meet with your care manager once a week for at least an hour for the duration of your involvement in Connect to Home. 

Where can I meet with my case manager? Can our meetings be virtual?

Services are provided in person or virtually, within your home or in the community. You may also attend group or individual sessions.

How much do Connect to Home services cost?

Connect to Home is free.

How do I get a referral for Connect to Home services?

You will need to get a referral from the Department of Children and Families Office of Housing & Department of Child Protection and Permanency Burlington East or West Local offices.

How do I qualify for Connect to Home Services?

To qualify for Connect to Home services, you must: 

  • Be between 18 to 21 
  • Be currently involved or have been in the custody of DCP&P Burlington Local offices
