Adult Residential Services provides safe, supportive housing for adults living with serious mental illness. Our goal is to help residents build daily living skills, increase independence, and work toward successful reintegration into the community. Through structured support, counseling, and life-skills training, we foster an environment that promotes recovery, personal growth, and stability. Our team partners with residents, families, and providers to deliver individualized, evidence-based care that helps prevent hospitalization and support long-term wellness.

Eligibility

To be eligible for Adult Residential Services, you must:

  • Be 18 years of age or older 
  • Have a chronic mental health condition 
  • Be transitioning from an inpatient or community-based setting and in need of supervised, supportive housing 

What to Expect 

  • Reaching Out

    After a referral is received, the client’s social worker will be contacted within 72 hours to review next steps and begin coordination.

  • Arranging Services

    Following the intake, housing placement will be secured and services will typically begin within 21 days of referral. 

  • Moving Forward with Care

    Clients will meet with their care team to develop an individualized treatment plan and daily living goals that support recovery, independence, and long-term stability.

How long can I stay in the program?

Length of stay depends on individual progress; residents may remain until they are ready to move to more independent housing.

How do I get referred to the program?

Most referrals come from hospitals or providers, but you can also call Acenda’s Access Center for more information. 

How do I qualify for Adult Residential Services? 

Adults 18+ with chronic mental illness who need supervised housing and support and are at-risk of hospitalization.

What can I expect while living in an Adult Residential Services home? 

Residents receive a structured, supportive living environment focused on recovery and independence. Your care team will work with you to develop daily living skills, manage your mental health, achieve personal goals, and build the confidence needed to transition to a more independent setting when you’re ready.

What services are available while I am in the program? 

Adult Residential Services provides individualized support based on your needs. Services may include case management, medication support, life skills development, behavioral health treatment coordination, recovery planning, and connections to community resources that promote long-term wellness and stability.

Can my family or support system be involved in my care? 

Yes. With your permission, family members or other supportive individuals may be involved in your treatment and recovery planning. Your care team encourages collaboration whenever appropriate to strengthen your support system and help you achieve your recovery goals.

What happens after I leave Adult Residential Services? 

Before you transition out of the program, your care team will develop a personalized discharge plan to help ensure a successful move to more independent living. This may include connections to outpatient behavioral health services, housing resources, medication management, and other community supports that promote continued recovery.

How is my treatment plan developed? 

Your treatment plan is created collaboratively with you and your care team based on your strengths, goals, and individual needs. Throughout your stay, the plan is reviewed regularly and updated as you make progress toward greater independence and recovery.

How will Adult Residential Services help me prepare for independent living? 

Your care team will help you build practical life skills such as managing daily routines, maintaining your health, accessing community resources, and strengthening independent living skills. The goal is to prepare you for a successful transition to the least restrictive and most independent living environment possible.
