Eligibility
To be eligible for PATH services, you must:
- Be over the age of 18
- Be experiencing or at risk of homelessness
- Have a Mental Health Diagnosis of Schizoaffective, Bipolar, Schizophrenia, Major Depressant, or Delusional Disorder
What to Expect

Getting a Referral to PATH
You can self-refer through Acenda’s Access Center, or receive a referral through any community program or provider working with you on housing.

Starting Your Relationship with PATH
You will be contacted by PATH within 72 hours of your inquiry. Path will schedule an in-person meeting to verify your eligibility and enroll you in services.

Moving Forward with Care
Once you have been enrolled, you will work with a PATH worker to create a treatment plan, set goals and schedule future case management meetings to ensure that you are supported throughout your journey.

How can I refer someone to your program?

Are you going to find me housing?
PATH case management makes every effort to connect individuals to appropriate housing and resources tailored to their individual circumstances. Due to the housing crisis, PATH must work conjointly with clients and other resources to have the best opportunity to find housing.
Does PATH pay for my housing?
No, PATH does not provide housing vouchers. PATH is a case management program designed to assist an individual with linking them to appropriate housing resources (vouchers), medical, and psychiatric support, in addition to other needed resources.
Can PATH help me avoid being evicted?
PATH is not legal representation, nor does PATH have funding to assist with potential eviction issues. However, PATH can assist, assess, and engage individuals experiencing or facing homelessness and utilize their wealth of knowledge to link individuals to all resources that can benefit them.
How do I qualify for PATH?
To qualify for PATH services, you must:
- Be experiencing homelessness or pending homelessness
- Be over the age of 18
- Have a Mental Health Diagnosis of Schizoaffective, Bipolar, Schizophrenia, Major Depressant, or Delusional Disorder
